Global Youth Car & Bus Permission Form

This form is to give consent for your young person to be transported in the car of a Global Youth Leader with their Green P's or full license, or a bus driven by a fully licensed Global Youth Leader.

Giving driving consent covers transportation:

To and from church

To and from youth events and connect groups

To and from church and youth related activities.

This form will be valid for one year from the submission date.
