Before completing this form an event request must be lodged (and approved) from your department. Please email Jeremy Crouch (jcrouch@globalheartchurch.com) to request an event.

1) Based on the church calendar and other Creative Department requests, we will do our best to accommodate you.                                            

2) Please ensure your creative request form is submitted with adequate time. In order for the Creative Department to review your request, and for it to be produced with excellence, we need a minimum of 6 weeks notice before you require the finished product.
